Provider Enrollment

Required documentation:

1 Completed application with certification fee
2 All applicable documents that apply to your company.

a Copy of the State License

b Copy of the Insurance certificate

c Copy of the company brochure (digital if available or scanned pdf)

d Completed IRS Form W-9

e If accredited copy of the same

Enrollment fee:

There is a one-time enrollment fee of $75.00 payable at the time of submitting the application. This fee is non-refundable.

Lead budget/fees

As a provider you will establish a monthly fee and will receive weekly reports as to expended and available balance for the current month. Although we are fully confident in our system to ensure you receive leads for services you provide and in the geographic area you selected...if you receive a lead to the contrary you will be granted full credit. You will receive a monthly report on each lead you were charged for including the date and time the lead was forwarded. Please note these are not exclusive leads and will be forwarded to multiple providers; thus, the end user has options. At the same time we will limit and certify only 5 providers per zip code for a specific service category.

Per lead fee as follows:

1 Home care services:
a  End user lead $20.00/per lead
b  Institutional lead $75.00/per lead


2 Durable Medical Equipment  (DME):
a  End user lead $50.00/per lead
b  Institutional lead $95.00/per lead
